Surgical Sperm Retrieval Techniques:

  1. PESA (Percutaneous Epididymal Sperm Aspiration):
    PESA is a minimally invasive procedure designed to extract sperm directly from the epididymis, the tube where sperm mature and are stored after being produced in the testicles. It’s most commonly performed when a man has obstructive azoospermia, where sperm production is normal, but there is a blockage preventing sperm from reaching the ejaculate.

      • A fine needle is inserted into the epididymis through the skin of the scrotum.
      • Sperm is aspirated directly from the epididymis.
      • Local anesthesia is usually used to minimize discomfort.
      • The procedure is performed under ultrasound guidance to ensure accurate placement of the needle.

      • Suitable for men with a blockage in the epididymis, but with normal sperm production.

  2. TESA (Testicular Sperm Aspiration):
    TESA is a surgical procedure used to retrieve sperm directly from the testicles. This technique is often employed when PESA fails, or in cases where sperm are not present in the epididymis. It is especially useful for men with non-obstructive azoospermia (where sperm production in the testicles is impaired but not absent).
    • Procedure Details:
      • A needle is inserted directly into the testicle under local anesthesia.
      • The sperm is aspirated from the testicular tissue.
      • In some cases, this is done using ultrasound guidance to find the best site for sperm extraction.
      • The procedure may be done as a single-step process or in combination with sperm extraction for IVF (In Vitro Fertilization).
    • Benefits:
      • Useful for men with non-obstructive azoospermia where sperm is still produced but may not be reaching the epididymis.
      • Quick and relatively low-risk procedure.
      • No need for general anesthesia in most cases.
    • Risks/Limitations:
      • Potential for damage to testicular tissue.
      • May not retrieve viable sperm if the sperm count is very low.
  3. Micro-TESE (Microdissection Testicular Sperm Extraction):
    Micro-TESE is an advanced and more precise surgical procedure for sperm retrieval, commonly used for men with non-obstructive azoospermia, where sperm production in the testicle is very low or absent. Unlike TESA, Micro-TESE involves the use of a high-powered microscope to identify the best areas of the testicle where sperm may be present, allowing for the extraction of sperm from very small areas of tissue.
    • Procedure Details:
      • The procedure involves a small incision in the scrotum to access the testicle.
      • A microscope is used to carefully examine the testicular tissue for sperm production areas.
      • Small amounts of testicular tissue are removed, which are then examined in the laboratory for sperm.
      • The procedure is performed under general anesthesia because it is more complex and involves tissue dissection.
    • Benefits:
      • Highest success rate for retrieving sperm in men with very low sperm production.
      • Highly effective for cases where previous sperm retrieval attempts have failed.
      • Can retrieve sperm even when sperm count is extremely low or absent in initial testicular samples.
    • Risks/Limitations:
      • More invasive and complex compared to PESA and TESA.
      • Higher cost and longer recovery time due to the need for general anesthesia and more extensive surgery.
      • Risk of damaging testicular tissue, which could affect hormone production and future fertility.

Which Procedure is Right for You?

  • PESA is generally used when sperm production is normal but there is a blockage preventing sperm from reaching the ejaculate.
  • TESA is typically recommended when sperm production in the testicle is present but sperm do not make it to the epididymis, or when PESA is unsuccessful.
  • Micro-TESE is reserved for cases with very low sperm production or when previous attempts at sperm retrieval have failed, as it provides the highest chance of success in these situations.

Post-Procedural Care:

  • Recovery Time:
    After PESA and TESA, patients can usually resume normal activities within a few days to a week. Micro-TESE may require a longer recovery period due to the invasiveness of the procedure.
  • Pain and Discomfort:
    Mild to moderate discomfort is common after these procedures, but it can be managed with pain medications. Swelling and bruising may occur, particularly with TESA and Micro-TESE, but it should subside within a few days.
  • Success Rates:
    Success rates vary depending on the technique used, the underlying cause of infertility, and the quality of sperm retrieved. Micro-TESE has the highest success rate for men with very low sperm production, while PESA and TESA have good success rates in cases of obstructive azoospermia.

Assisted Reproduction Following Sperm Retrieval:

Once sperm is retrieved, it can be used in assisted reproductive techniques such as:

  • ICSI (Intracytoplasmic Sperm Injection):
    The retrieved sperm is injected directly into the egg to facilitate fertilization.
  • IVF (In Vitro Fertilization):
    Sperm retrieval via any of these techniques can be followed by IVF, where the eggs and sperm are combined in a laboratory to create embryos, which are then transferred into the uterus.
